Consent forms

Before any treatment, you should know what it involves, what can go wrong and what the alternatives are. Each page below sets that out in plain English — read whichever applies to you, whether or not you're booked in with us.

Signing online, and what it means

If your dentist has asked you to complete a form before your appointment, you can read it and sign at the bottom of the page on your phone — no printing, no scanning.

Signing online confirms you've read and understood the information. It isn't consent on its own. Your dentist will still go through it with you and confirm your consent in person before any treatment starts, and you can change your mind at any point up to then.

If you'd rather read it on paper, or you'd like to go through it with us over the phone first, call 07 4635 1444 — that's completely fine and it's how it has always worked.
